Job Description

Key: HCP- Home Care Personnel.

• Supervision of HCP: Supervise and oversee the work of home care personnel, to ensure care delivery is appropriate and meets professional and regulatory standards. Conduct initial Visits, supervisions and reassessment as needed.
• Care Coordination: Collaborate with the Director of Nursing, other healthcare providers, and support staff to ensure patients receive comprehensive, individualized care plans based on the doctor’s orders.
• Patient Assessments: Conduct in-home assessments and reassessments of patients’ conditions, reviewing and modifying care plans as necessary. Ensure accurate and timely documentation of assessments.
• Compliance and Quality Assurance: Ensure compliance with state and federal regulations, accreditation standards, and agency policies. Assist in conducting quality assurance checks, audits, and reviews.
• Training and Development: Provide training, coaching, and continuing education to patients and HCP on agency policies, procedures, and best practices in home health care.
• Interdisciplinary Communication: Facilitate clear communication between HCP, patients, families, and the agency to promote collaborative care.
• Performance Evaluation: Participate in the performance evaluation of HCP, offering feedback and identifying areas for improvement.
• Document Management: Ensure all applicable forms are completed, clear, legible, and entered into the correct database.

 
Requirements:

• Bilingual: Spanish
• Proficiency in electronic health record systems and documentation.
• Ability to travel within the agency's service area for patient visits and staff supervision. (Manhattan and the Bronx).
• Flexibility to work weekends and evenings, if required.

Physical Requirements:

• Ability to lift, push, or pull up to 50 lbs.
• Prolonged periods of standing, walking, and bending during patient care activities.
