Job Description

We are looking for a board-certified Family or Internal Medicine Physician to join our esteemed team in Derby, CT , and surrounding areas. In this role, you will provide high-quality, patient-centered care in an office-based setting, managing both acute and chronic conditions while working collaboratively with a dynamic team of healthcare professionals.

Key Responsibilities:

• Patient Care: Examine patients, obtain medical histories, and diagnose and treat both acute and chronic illnesses.
• Diagnostic Expertise: Order, perform, and interpret diagnostic tests to assist in the treatment and management of various health conditions.
• Care Plans: Develop comprehensive treatment plans, involving patients and their families in the decision-making process.
• Specialty Referrals: Initiate appropriate referrals to specialists when needed to ensure complete care for your patients.
• Collaborative Approach: Work closely with the care team to improve service delivery, address care gaps, and enhance quality measures.
• ACO and PCMH Initiatives: Actively participate in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives.
• On-Call Coverage: Provide on-call coverage within the primary care group as needed.
• Compliance and Documentation: Ensure accurate patient documentation, and work with management to ensure compliance with billing and coding standards.

Qualifications:

• Education: Graduation from an accredited medical school (MD or DO).
• Residency: Graduation from an accredited Family Medicine residency program.
• Licensing: Valid and unrestricted Connecticut medical license (or ability to obtain).
• Certification: Board-certified in Family Medicine.
• Experience: DEA registration and Controlled Substance Certificate.
